HPV vaccine catch-up for women aged 27–45 has grown from off-label to mainstream guidance. The original target group was girls aged 9–14, but newer evidence supports vaccinating older adults — especially those not yet exposed to the major oncogenic HPV types.

Who is eligible for catch-up?
Indian and international guidelines now support HPV vaccination up to age 45 for women who are sexually active but have not yet been infected with the major oncogenic HPV types covered by the vaccine. Eligibility is by shared decision-making with your doctor, not a strict cut-off.
What does the evidence show?
The benefit is smaller in catch-up populations than in pre-exposure vaccination — but it is real. Pooled analyses suggest the 9-valent HPV vaccine reduces incident HPV infection by 30–60% even in mid-adult women who may have prior exposure to other HPV types.
Dose schedule and cost
For ages 15 and above, the 9-valent vaccine is given in a 3-dose schedule (0, 2, 6 months). Cost in India ranges Rs 3,000–6,000 per dose depending on brand and pharmacy. Many corporate health-insurance policies do not yet cover adult HPV vaccination; it is typically self-paid.
What the vaccine does NOT do
- Does not treat existing HPV infection or precancerous lesions.
- Does not cover every high-risk HPV type — screening (Pap smear or HPV DNA test) is still essential.
- Does not protect against established cervical cancer in any way.

FAQs
I'm 38 and married. Is the HPV vaccine still useful?
Probably yes, particularly if you have had limited sexual partners or have not previously had HPV-related abnormal Pap smears. The benefit is smaller than at age 14, but the safety profile is excellent. Discuss with your gynaecologist.
Do I need HPV testing before getting vaccinated?
Routine pre-vaccination HPV testing is not required. If you have a recent abnormal Pap smear, get the abnormal area evaluated and treated first.
Are 2 doses enough for catch-up?
Three doses are recommended for ages 15 and above. The 2-dose schedule is only validated for the 9–14 age group.
Is the vaccine safe in pregnancy?
HPV vaccine is not recommended during pregnancy. Defer until after delivery.
Can my husband also get HPV vaccinated?
Yes — HPV vaccination is approved for males up to age 45 in many regions. It reduces the risk of anal, penile and oropharyngeal cancers and also reduces transmission to partners.
