Colposcopy: The Diagnostic Foundation
Colposcopy is the first step after any high-grade smear result or positive HPV with abnormal cytology. The colposcopist examines the cervix under magnification after applying dilute acetic acid and Lugol iodine solution. Acetowhite changes and mosaic or punctation vascular patterns indicate areas of higher-grade CIN. Targeted biopsies are taken from the most abnormal-appearing regions, and the results determine whether treatment is needed.
LEEP/LLETZ: The Standard Treatment for High-Grade CIN
Large loop excision of the transformation zone (LLETZ in the UK, LEEP in North America) uses a wire loop energised with electrical current to excise the entire transformation zone under local anaesthetic. The procedure takes 10–20 minutes. Recovery involves a few weeks of light discharge and avoiding intercourse and tampons. The excised specimen provides definitive histology — confirming the grade of CIN and whether margins are clear. Cure rates for CIN 3 with a single LLETZ are 90–95%.
Cone Biopsy: When More Tissue Is Needed
A cone biopsy removes a cone-shaped section of the cervix under general or spinal anaesthesia. It is used when: the lesion extends into the endocervical canal beyond the reach of LLETZ; when glandular abnormalities (CGIN/AIS) are present; or when early invasive disease needs to be excluded with clear deep margins. The procedure slightly increases the risk of cervical incompetence in subsequent pregnancies — an important consideration for women who have not yet completed their family.
After Treatment: Surveillance Matters
Treatment is not a discharge point — it is the beginning of a surveillance programme. Test-of-cure co-testing at 6 months distinguishes women who are HPV-negative (low risk, return to routine screening) from those who are HPV-positive (higher risk, annual surveillance for 3–5 years). Persistent HPV after two treatments warrants discussion about further management options including hysterectomy in women who have completed their family.
