Uterine fibroids symptoms — Fibroids: The Most Common Tumour in Women

Uterine fibroids — also called leiomyomas or myomas — are non-cancerous growths of the uterine muscle wall. They are the most common benign tumour in women of reproductive age worldwide: by the age of 50, an estimated 70–80% of women will have at least one fibroid, though many will never know. Despite being universally benign, fibroids cause significant morbidity and are the leading indication for hysterectomy in India and globally.

Types and Location Matter

Fibroids are classified by location within the uterus, and location largely determines symptoms:

  • Submucosal — project into the uterine cavity; even small submucosal fibroids cause heavy bleeding and fertility problems
  • Intramural — within the uterine wall; the most common type; cause bulk symptoms and heavy periods when large
  • Subserosal — project outward from the uterine surface; cause pressure symptoms (urinary frequency, constipation, back pain)
  • Pedunculated — attached by a stalk; can twist (torsion), causing acute pain

Symptoms That Should Not Be Normalised

Many women with fibroids are told their heavy periods are “just hormones” or that their pelvic pressure is “normal for their age.” Symptoms that are caused by fibroids and deserve evaluation include:

  • Periods lasting more than 7 days or soaking through a pad/tampon every 1–2 hours
  • Iron deficiency anaemia from chronic blood loss
  • Pelvic heaviness or a feeling of pressure in the lower abdomen
  • Urinary frequency or urgency (fibroid pressing on the bladder)
  • Constipation or incomplete emptying (posterior fibroids pressing on the bowel)
  • An abdomen that appears larger than expected
  • Recurrent pregnancy loss or difficulty conceiving

Are Fibroids Ever Cancer?

Standard uterine fibroids do not transform into cancer. Uterine sarcoma (leiomyosarcoma) is a separate, rare malignant tumour that can occur in the uterine muscle. The risk of a uterine mass being sarcoma rather than fibroid is approximately 1 in 500 — very rare, but not zero. Rapidly growing fibroids, particularly in postmenopausal women, warrant specialist assessment. Any new uterine mass in a postmenopausal woman should be evaluated by a gynaecological oncologist.
